HIPAA Notice of Privacy Practices

Optimal Clinical Research is committed to protecting the privacy of your protected health information ("PHI") in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state and federal laws.

For treatment: to provide medical care during your participation in a clinical study.

For payment: to bill sponsors or other authorized parties for study-related services.

For research: to conduct clinical research studies you have consented to participate in, and to share required information with study sponsors, CROs, IRBs, and regulatory agencies such as the FDA.

As required by law: when disclosure is required by federal, state, or local law.

You have the right to inspect and receive a copy of your medical records, request corrections, request restrictions on certain uses or disclosures, receive an accounting of disclosures, request confidential communications, and receive a paper copy of this notice.

You have the right to withdraw your authorization to use or disclose your PHI at any time, in writing, except to the extent we have already relied on it.

We are required by law to maintain the privacy of your PHI, provide you with this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
