This notice describes our privacy practices as they apply to information collected through this website and through research-related activities. For treatment: to provide medical care during your participation in a clinical study.

For payment: to bill sponsors or other authorized parties for study-related services.

You have the right to inspect and receive a copy of your medical records, request corrections, request restrictions on certain uses or disclosures, receive an accounting of disclosures, request confidential communications, and receive a paper copy of this notice.

You have the right to withdraw your authorization to use or disclose your PHI at any time, in writing, except to the extent we have already relied on it.

We are required by law to maintain the privacy of your PHI, provide you with this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
